Enrolled Camper Forms:
MUST BE TURNED IN BEFORE THE START OF CAMP
Can you believe camp starts in 2 weeks? We can't! Below you will find links to all the forms we'll need from each camper and family! We need all of these forms returned together, prior to the start of camp. Campers will not be able to attend camp without all forms on file. All of the forms can be sent to us via email!
Medical Form- This form can be combined with your child's most up to date medical records. If your camper attended camp last summer, we can use the form from last summer until you are able to get the most recent ones.
Permission Form- Grants Camp Kef permission to take photographs of your child and use your family as a reference.
Adult Release & Waiver of Liability- Must be signed by every adult living in the house with the child(ren) attending camp. All adults can sign the same form.
Minor Release & Waiver of Liability- You can put up to 5 children on this form, which means we can accept one per family.
A note about payments:
- All camp payments are due in full before 6/20.
- If the payments are unable to be processed, you will receive an email from our Accounting Department.
- Campers will not be able to attend camp unless all payments have been made.
- For any information about bills, balances, or anything finance related, please contact Julie at email@example.com. We have no access to any camp accounting information!
