I hereby give Essential Utilities Services, Inc. and any of its related legal entities (collectively “Essential Utilities”) and any third party with whom Essential Utilities deems appropriate the right and permission, with respect to any and all likenesses, photographs, video or audio recordings taken of me or in which I may be included with others for the following purposes:

(a) To copyright any and all likenesses, photographs, video or audio recordings in the name of Essential Utilities or under any other name Essential Utilities may choose, including third parties;

(b) To use and re-use any and all likenesses, photographs, video or audio recordings in whole or part, individually or in conjunction with other photographs, video or audio recordings in any medium, including but not limited to tv, radio, print or social media for the purpose of education, illustration, promotion, marketing and advertising; and

(c) To use my name in connection with the use of any and all likenesses, photographs, video or audio recordings as Essential Utilities may choose to do.

I hereby release and discharge Essential Utilities from any and all claims and demands arising out of or in conjunction with the use of the likenesses, photographs, videos and/or audio recordings taken of me, including any and all claims for libel.

I am 18 years of age or older.

NOW THEREFORE, for good and valuable consideration, the sufficiency of which is hereby acknowledged, the parties, intending to be legally bound, agree to the above.

I have read the foregoing and fully understand the contents thereof.
